The form is quite specific about where you started your journey from and where you pass through.

Returning via Athens you should have enough time (it has to be less than 48 hours) to fill it in before leaving Crete.

We had to have AMKA numbers for our beige permits, so we have these.

The implication was that it was in all cases - but I don't know.
We were not planning to use an exemption certificate.

I agree with the advice given in this thread, particularly to sort it out first. Checked after passport control at Chania and Heraklion too, of course, so need to be ready.
